Overload occurs when the rate of joint loading exceeds the tissue’s ability to recover. This is the core mechanism behind PFP among single-sport athletes.
How does single-sport specialization lead to this?
Athletes who play only one sport, or individuals who engage in only one type of physical activity, repeat the same joint loading pattern constantly, without the varied movement that multi-sport/multi-activity participation provides. Combined with minimal recovery time, this is a primary etiologic factor for PFP.
What happens physiologically?
Overload is evidenced by increased patellar water content and elevated bone metabolic activity, both signs that the joint and surrounding bone are under significant, measurable stress.
Who is most affected?
Adolescent athletes show a 1.5x greater risk for PFP when specializing in a single sport. This pattern also shows up clearly in military training, where overload is a well-documented etiologic factor.
